The Carmela Sopranos are spending time at the museum when Carmela begins to notice blood. She asks for a tampon and goes to the ladies' room; when she returns, Meadow is looking at the paintings. Carmela begins to tear up when she sees the painting "The Mystical Betrothal of St. Catherine" by José de Ribera. Later, while watching something sentimental on TV, Carmela begins to cry, but quickly regains her composure when she realizes that it is a dog food commercial. She later confesses to a priest who plans to pursue a doctorate in psychology. Carmela tells the priest that she is worried that she has ovarian cancer and also tells him about her meeting with Dr. Krakower. The priest asks Carmela if she loves Tony, to which she replies: “I do.” He then advises her not to leave him, but to help him grow up, be more loving towards him, and "learn to live with the good side." Over lunch with the other gangster wives, Carmela tells them that she is perfectly healthy, and that she was afraid she was pregnant, but she turned out to have thyroid problems. Rosalie April tells them that they should all admire Hillary Clinton, not for her personality, but for the way she acted when Bill Clinton admitted to their affair. Carmela agrees and says, "She should be a role model for all of us."

Tony continues to see Gloria Trillo even as her erratic behavior continues. She apologizes to him in the garage below Dr. Melfi's office, but Tony replies, "If you were a guy, I wouldn't have to tell you where you would be right now." Gloria asks for forgiveness and wants to make their relationship work, but Tony needs time to think. When he tells Dr. Melfi about his fiery love-hate relationship with Gloria, she uses the term "amour fou" [mad love]. Melfi then tells Tony that Gloria loved him. Tony decides to give the relationship another chance, but leaves when Gloria blows him off because her tires were slashed. He later finds out that she gave Carmela a ride home from Globe Motors, and he finally breaks up with her. She later calls him, distraught at the thought of being incoherent, and he visits her, but their conversation soon turns into a heated argument. He slaps her and she mockingly says, “You’re bad,” and that she should “sit there dumb” while he has sex with other women. With those familiar lines, Tony suddenly realizes that Gloria is a lot like his mother ("I've known you my whole fucking life"). Gloria also derides Carmela as a "goomba housewife" who will put up with anything in exchange for a big, "tacky" ring. As Tony walks out the door, Gloria threatens to tell Carmela about cheating if he leaves. Enraged, he attacks her, overturning her dining table and throwing her across the room. The fight continues and he begins to choke her. She begs him to kill her, but he leaves instead.

Jackie Aprile Jr. and his friend Dino Zerilli want to succeed in life and become more than just accomplices. After hearing Ralph Cifaretto's story of how Tony and Jackie Aprile Sr. made a quickie out of Fitch Lamanna's Saturday card game, the two decide to do the same during Eugene Pontecorvo's game, asking Carlo Renzi to join them because he has a shotgun. Arriving at the hangout, Jackie is embarrassed and decides to back down, even though he was the one who suggested the idea. But Dino convinces him to go through with it "before the pen wears off." The three don ski masks and enter Team April's hangout where a poker game is taking place, but they are alarmed when the players include familiar faces: Christopher Moltisanti, Furio Giunta and Ellie Boy Barese. Carlo and Dino demand that the players give them their money and stay calm while Jackie tries (and fails) to remain silent for fear that his voice will be recognized. The dealer, "Sunny", continues to outbid would-be robbers and is shot and killed by a panicked Jackie. As the gangsters draw their guns, a shootout breaks out: Jackie shoots Furio in the thigh, and Christopher shoots Carlo in the forehead, killing him instantly. Matusz, Jackie's driver, leaves the robbers as soon as he hears the first shot, and as Jackie and Dino run outside, Jackie steals a passing car, leaving Dino to be caught and executed by Ellie Boy and Christopher. Furio was quickly rushed to the office of Dr. Fried, a urologist, for undercover emergency surgery. In the waiting room, Christopher warns Tony that he knows Jackie Jr. was an escaped robber, and that he must be killed. However, Tony resented his long history with Jackie Jr.'s father.

Patsy Parisi is then shown taking Gloria for a test drive in a Mercedez-Benz. He pulls over to the side of the road in an isolated area and waves a gun, telling her that she and Tony are over. When she tries to protest, he stupidly tells her to shut up and listen: she can either stay away from Tony or the story will go where they "scrape [her] nipples off those fine leather seats." He also tells her that her death will be unpleasant, not like in the movies, and that the last face she sees will be his, not Tony. He then gets into his Cadillac, which was already parked at the spot, leaving her dazed and shaking in the test car. Patsy is later shown having a trivial conversation on her cell phone about groceries and driving away uneventfully in her car.
